LEGAL INFORMATION: The undersigned (hereinafter referred to as "Customer") requests that Dallas-PWD, Inc (hereinafter referred to as "Pella") sells to the customer goods on account.

1.) The Customer hereby grants permission now and from time to time while Customer is doing business with Pella, for Pella and/or its agents to verify and/or supplement the information stated hereon, including but not limited to inquiries of Customer's credit references and obtaining credit reports on the Customer, in the sole discretion of Pella.

2.) Completion of this request for information does not imply or guarantee extension of terms. Each invoice provided by Pella to Customer governs the terms and conditions of any credit extended to Customer from the date next to the Customer's signature. These terms are not a guarantee or assurance of any extension of credit and Pella is not obligated to extend credit at any time and may grant, revoke, or modify the terms of credit at any time.

THE CUSTOMER ACKNOWLEGES AND UNDERSTANDS PELLA WILL USE THE SERVICES OF CREDIT REPORTING FIRMS TO VERIFY AND REPORT PAYMENT HISTORY. A FACSIMILE OR DIGITAL COPY OF THE AGREEMENT SHALL BE BINDING AS AN ORIGINAL DOCUMENT SIGNED AND DELIVERED TO PELLA.
